If you would like to register your animal with our Practice, please call the surgery on 01296 623439 or call in to either the Halton or Merrilaw Surgeries and give the receptionist your details and the details of your animal(s).

 It’s useful to register even if you don’t require an appointment for your animal at this time - when you do need to make an appointment or call us in an emergency, it means we already have your details to hand.

Alternatively, you can follow the link below  and submit the form via e-mail to us. Before  submitting the form, please refer to our Terms and Conditions of Business. You can also use the form to register additional animals if you are already a client of the practice. (There is a separate Equine Registration Form  in the Equine  section of this site if you would prefer to use this for equine registrations.)

If you are moving from another veterinary practice we will need to obtain any records they hold for your animals.  We will contact them and arrange this, however, they should also contact you to let you know that we have made the request to transfer records.  If your animal needs veterinary treatment in the meantime, however, before receipt of the records, it will of course receive whatever care and treatment is necessary.
